All forms are HIPPA-compliant, and you will be able to download a PDF copy of your records once they are completed. Laboratory
Form
As part of your treatment plan, certain laboratory tests will be required to manage your health. Some of these tests may need to be performed by an outside lab. To ensure that you receive the best care possible while also taking advantage of your insurance coverage and benefits, please take some time to fill out the information on this laboratory form. When you are able to provide accurate and up-to-date details, it helps our team here at HOAF coordinate your care efficiently and minimize any potential delays in processing your lab work.
New Patient And
Family History Form
At HOAF, we aim to build a comprehensive profile for you in order to have a complete picture of who you are and any prior medical conditions you may have experienced. By learning in detail about your previous health conditions, we are able to screen, treat and manage any potential risks more adequately. Additionally, understanding your family health history is crucial in cancer prevention, early detection and efficient treatment planning. If our team is aware about the cancers that have impacted your family members, it allows us to determine whether you may be at risk for developing that same disease. As a result, our medical oncologists are able to help assess whether you would benefit from genetic counseling, genetic testing for certain cancer genes or frequent monitoring. We also understand you may not know certain details about your immediate family and that is ok—please fill out the forms to the best of your ability.

Packet
We understand filling out paperwork is a tedious task, but it is necessary to ensure you receive the best treatment possible. At HOAF, we take a collaborative approach to make certain that there are no gaps in your healthcare coverage. That’s why it is critical you fill out our new patient packet—so we are able to gather all the necessary information to start your treatment with us at HOAF.
To make it easier, we recommend collecting all the information you need before beginning to fill out this packet. You will need items such as your health insurance card, prescription information and a valid photo ID.
We also ask that you tell us about the providers or specialists you currently see, including their names, practice locations, phone and fax numbers, to streamline any communication in regards to your care.
Release Of
Information Form
To ensure seamless continuation of care, we often need to obtain medical records from other physicians or hospitals. Please complete this release form for any healthcare provider that may have records that are relevant to your treatment with us.
If you need to send records to another physician, attorney or insurance group, you can email the completed form back to us.
For personal copies of your records, please bring the completed form and a valid ID to our office. If you are unable to make a trip to our office in-person, you can mail the form to The Lodge at 4501 Empire Ct., Fredericksburg, VA 22408.
